Cislee 10/50 Injection

Generic Name– Cisplatin
Drug Class– Antineoplastic agent
Description- Cisplatin is a chemotherapeutic drug, which has been used for treatment of numerous human cancers including bladder, head and neck, lung, ovarian, and testicular cancers. It is also effective against various types of cancers, including carcinomas, germ cell tumors, lymphomas, and sarcomas. It acts by cross linking with the purine bases on the DNA, thereby disrupting DNA repair mechanisms, causing DNA damage, and inducing apoptosis in cancer cells.

Indications and Usage

Cisplatin is indicated for the treatment of

  • Advanced testicular cancer
  • Advanced ovarian cancer
  • Advanced bladder cancer

Limitations of Use
Cisplatin has few indications for treatment in children, and utilized for historically aggressive tumors

Dosage and Administration

BEP (Bleomycin, Etoposide and Cisplatin) Regimen
Bleomycin 30 units IV weekly on Days 1, 8, and 15 or Days 2, 9, and 16
Etoposide 100 mg/m2 IV on Days 1–5
Cisplatin 20 mg/m2 IV on Days 1–5

Repeat every 21 days

VIP (Etoposide, Ifosfamide and Cisplatin) Regimen
Etoposide 75 mg/m2 IV on Days 1–5
Mesna 240 mg/m2 IV over 15 minutes before ifosfamide, then at 4 and 8 hours from the start of each ifosfamide dose daily on Days 1–5
Ifosfamide 1200 mg/m2 on Days 1–5
Cisplatin 20 mg/m2 IV on Days 1–5

Repeat every 21 days

TIP (Paclitaxel, Ifosfamide and Cisplatin) Regimen
Paclitaxel 250 mg/m2 IV on Day 1
Ifosfamide 1500 mg/m2 IV on Days 2–5
Mesna 300 mg/m2 IV over 15 minutes before ifosfamide, then at 4 and 8 hours from the start of each ifosfamide dose daily on Days 2–5
Cisplatin 25 mg/m2 IV on Days 2–5

Repeat every 21 days

EP (Etoposide and Cisplatin) Regimen
Etoposide 100 mg/m2 IV on Days 1–5
Cisplatin 20 mg/m2 IV on Days 1–5
  • Dosage Regimen for Ovarian Cancer: Cisplatin 75 mg/m2 to 100 mg/m2 per cycle once every 3 to 4 weeks followed by cyclophosphamide 600 mg/m2
  • Recommended dose in advanced bladder cancer: 50 mg/m2 to 70 mg/m2 intravenously per cycle once every 3 to 4 weeks

Dosage Forms and Strengths

  • Supplied as a single use vial for intravenous infusion
  • Available as 10mg and 50mg for injection

Special Precautions

  • Patient must achieve adequate hydration and must maintain hydration and urinary output for 24 hours after administration.
  • Always administer pre- and post-treatment antiemetic to prevent nausea and vomiting
  • Consider dose reductions or alternative treatments in patients with renal impairment as cisplatin can cause severe renal toxicity including acute renal failure
  • Monitor patients for signs and symptoms of peripheral neuropathy and infection caused due to severe myelosuppression
  • Consider audiometric and vestibular function monitoring to determine ototoxicity in pediatric patients
  • Monitor patients receiving cisplatin for possible hypersensitivity reactions and injection site reactions
  • Administer by slow intravenous infusion and avoid contact of cisplatin with aluminum parts
  • Determine pregnancy status prior to initiation of cisplatin and advise women of reproductive potential to use effective contraceptives to avoid pregnancy
  • It is not recommended in pregnant and breast feeding mothers, as it can be harmful to fetus
